This inspection included evaluation of the status of previous inspection findings and manufacturing process control. Additionally, a followup was performed in regard to:

UNRESOLVED ITEMS:

Sixteen clevises for B-P Job No. 8578-0011-01 (Beaver Valley, Unit 2) contained nonconforming welds and were on hold for disposition.

Final disposition and documentation by B-P were halted due to the customer's, Applied Engineering Company (AEC), stop work order on the job.

This matter is considered unresolved pending determination of final disposition and NRC review.

STATUS OF PREVIOUS INSPECTION FINDINGS:

1.

(Closed) Nonconformance A (81-02): Noncompliance with the requirements of paragraph NF-4231.2 in Section III of the ASME Code concerned the use of we-1ded temporary attachments during assembly of end attachments (brackets) for restraint devices.

The NRC inspector verified that in accordance with the vendor's response letter dated April 27, 1982, use of temporary tie bar attachments for maintenance of alignment was limited to welding assembly of the bracket side pieces.

This practice was discontinued in November 1981 and documented by QA memorandum to production. As an alternate alignment method, mechanical jigs were i

developed for the various size brackets. The welding alignment j

jigs are now being used during welding of brackets.

(Closed) Nonconformance B (81-02):

This noncompliance with the requirements of paragraph NCA-3820(b) in Section III of the ASME Code concerned qualification / certification of a material supplier /

manufacturer (Marmon-Keystone Corporation) who was awarded purchase orders for ASTM A-500 Grade B tubing.

PAGE 3 of 6 The NRC inspector verified the statements in B-P's letter dated April 27, 1982, with respect to paragraph NF-2610(b) in Section III of the ASME Code being the applicable requirement, and that paragraph NCA-3820(b) was not applicable to the subject material.

No nonconformance therefore occurred.

B-P was determined to have met the requirements of NF-2610(b) with respect to the Marmon Keystone Corporation plants located in Worcester, Massachusetts, and Butler, Pennsylvania, by the following three methods:

(a) recognition as an ASME Certificate Holder; (b) audit / survey by B-P; and (c) random tests on materials.

In recognition of the need to clarify the measures for compliance with NF-2610(b) requirements, B-P performed the following actions:

(a) developed Procedure No. BP-11-7, Supplement A, Revision 1, to directly address vendor verification; (b) issued Quality Directive No. N-5 to include control of vendor audits in the next revision of the QA Manual; (c) revised their Procedure No. BP-7-5 which controls vendor rating and qualification; and (d) reworked their vendor list to include qualification and restrictions applied to each vendor.

OTHER FINDINGS OR COMMENTS:

1.

Manufacturing Process Control:

The NRC inspector reviewed the B-P QA Manual, Revision 2, dated July 24, 1981, and the manual QA Directives (Nos. 1-13) to verify that this activity is controlled by the QA program.

Observations were made in the shop of 16 clevis assemblies for B-P Job No. 8578-0011-01.

The clevises were on hold due to nonconforming welds, and the lot container was tagged with B-P Disposition Tag No. 13007, dated May 12, 1983.

The NRC inspector also examined the following documents which were applicable to B-P Job No. 8578-0011-0, pipe restraint assemblies for Beaver Valley, Unit 2:

(a) fabrication traveler record package for Part No. 72042, NF Clevis, Item Mark Nos. 2 MSS-PRR-836 and -846; (b) fabrication traveler record packages for Part No. 72100, Restraint Straps; (c) fabrication traveler record packages for pin subassemblies; (d) magnetic particle (MT) Procedure No. BP-9-5, Supplement B, Revision 7; (e) SNT-TC-1A qualification records for a B-P Level II NDE technician; (f) Welding Procedure Specification No. WPS-1-11-2; and (g) two AEC corrective action request (CAR) reports.

PAGE 4 of 6 With respect to the work for Beaver Valley, Unit 2, AEC had issued a stop work order and CAR Nos. 56 and 57 dated May 4,1983.

This action was based on work on the clevises which AEC considered unacceptable.

As a result, B-P had ceased all activiites on the work including documentation.

In general, AEC's concerns with clevis work resulted from observations by AEC representatives of weld surface defects and unsatisfactory performance by a B-P NDE technician of a MT examination, and also questions concerning B-P's use of the appropriate approved revision levels of NDE procedures.

During record review and observations of the 16 nonconforming clevises, the B-P QA Manager brought to the NRC inspector's attention that the 4 longitudinal butt welds in each clevis had been improperly made by B-P.

These welds were required by the traveler fabrication sketches to be full penetration vee groove welds.

Instead, full penetration single-bevel welds had been made, and as a result B-P had tagged the items with a Disposition Tag 'and recommended that all 16 clevises be scrapped. However, they were unable to complete and document the disposition in a nonconformance report due to their customer's stop work order. The NRC inspector verified that the above conditions existed and that the nonconforming weld design had not been identified in the two CARS issued by AEC.

The final B-P disposition action and documentation of the clevis welding nonconformance will be reviewed during a subsequent NRC inspection.

This condition is considered an unresolved item.

2.

Unidentified Welds on Struts at Wolf Creek Station:

==

Introduction:==

This inspection was made in followup of a potential 10 CFR Part 50.55(e) report dated March 25, 1983, by Kansas Gas and Electric Company (KG&E) concerning the identification at the Wolf Creek site of defective welds and the presence of welds not shown on the fabrication drawings in 12 of 40 B-P sway struts.

All of the weld deficiencies were examined at the site by a B-P representative.

On May 26, 1983, KG&E notified the NRC by letter that the majority of the sway strut welds were evaluated and no adverse safety concerns were identified. A few welds remained to be evaluated.

Findings: During this inspection, the NRC inspector independently verified by document review that the incident involved the presence of miscellaneous surface weld defects and unidentified closure welds

The defects had been examined by representatives of KG&E, Daniel Construction, Bechtel Corporation, and B-P, and were documented in Daniel Construction Nonconformance Report (NCR) No. ISN7810PW.

B-P, at the written request of Bechtel, provided their written evaluation of the weld deficiencies.

This evaluation indicated that the weld deficiencies were nondetrimental to safety, even though some were not in compliance with B-P shop fabrication drawings.

While at B-P, the NRC inspector determined that the strut paddle fabrication drawing required only four vertical fillet welds joining the paddle to the shaft. Unspecified horizontal fillet welds between the paddle and shaft had been added.

Review of randomly selected welding and visual inspection fabrication records for paddle assemblies for 70, 120 and 200 KIP sizes of struts did not reveal any significant deficiencies.

B-P's disposition of the unspecified weld was occumented in B-P NCR No. 14 dated December 17, 1982.

Included with the NCR was the completed B-P CAR No. 83-1 which indicated that additional training of welders had been performed to prevent recurrence of the strut paddle welding problem.

Discussion and document review also indicated that nonconforming welded strut paddle assemblies had been furnished to the Shearon Harris Nuclear Power facility and other unidentified sites.

The B-P QA Manager indicated that since their evaluation indicated that the strut-paddle weld nonconformances were not safety-related, B-P did not make a review to determine what other nuclear sites may have received these items.

No nonconformances were identified in this area of inspection.

3.

Misapplication of EA-3 Clamps:

Introduction:==

This inspection was made in followup of a B-P potential 10 CFR Part 21 report concerning the possible misapplication at Brunswick, Units 1 and 2, of EA-3 strut restraint clamps that were used in conjunction with hydraulic shock suppressors.

This matter was also identified in the NRC Information Notice No. 83-13.

l The event which prompted B-P to report this matter was receipt of a i

letter dated February 9, 1983, from United Engineers and Constructors,

~

Inc. (UE&C) concerning evaluation of the use of EA-3 clamps in an l

off-axis design condition at Brunswick, Units 1 and 2.

This letter indicated that the clamps were overloaded.

PAGE 6 of 6 It appeared that UE&C who had design responsibility had misinterpreted a statement in the B-P Catalog No. 66, page 45, which indicated that the snubber units could be used in any position.

As a result of this notification, B-P performed a review of all utility nuclear units where the EA-3 clamps could potentially have been misused. At the time of the NRC inspection, a review and notification had been made by B-P to 9 nuclear sites where EA-3 clamps may have been used and for which B-P did not have design responsibility.

B-P also participated in a generic review and notification action concerning 21 nuclear power generating units for which B-P did have design responsibility.

I In addition, B-P had identified and reviewed 6 additional units for which they had design responsibility, but which had not been included in the original notification to the NRC.

These units were in preliminary design stages during B-P's change in Part No. EA-3 to Part Nos. 2600 and 2602. The 2602 part was designed for off-axis loading.

No design deficiencies were identified by B-P in these 6 units.

The status information on the above actions was transmitted by letter dated March 30, 1983, to the NRC Office of Inspection and Enforcement which stated that a final report would be made by B-P to the NRC when all of the ongoing reviews and notifications are complete.

Further review by the NRC inspector indicated that at the time (prior to 1977) of the use of the EA-3 clamps, there was no specific application information furnished by B-P for these clamps, other than in Catalog No. 66.
