AFFIDAVIT OF ARVIL DILLINGHAM, JR.

Q: Please state your name and address.

A: Arvil Dillingham, Jr. ("J.R."), P. O. Box 124,' G'len Rose, Texas 76043.

Q: Since your last affidavit, is them additional information you have mmenbered or have knowledge about regarding the Comanche Peak nuclear power plant?

A: Yes. I have personal knowledge of improper welding of the stainless steel liners. Instead of having a backing strip with a 3/16" to 3/8" gap,

there was just a butt weld and a heliarch wire for a fil1et. I also know of improper installment of the CCW surge tanks. While instal: ling the tanks, the anchor bolts wem out of alignment with the holes in the base plate of the tanks, and instead of plug welding the holes up in the base plate and mdrilling to fit the anchor bolt pattern, the anchor bolts wem bent over to fit the holes.

Therefore, when they tried to put the tanks in the proper location, the bent studs became binding in the hole. This caused the bent stud to become cut on one side and the straight studs to stmtch from so much pmssure being applied.

Also, the torquing was improper.

Q: Was this safety-related ("Q")?

A: Yes. Both of them were safety-mlated.

Q: Are there other concerns?

A: Yes. I was also told of improper bolting in the upper lateral restraint; for instance, the bolts were supposed to be 7" long. When they couldn't get them in, they cut about 3" off and now they have a 2" diameter bolt with only a pene-tration of 1/2" inside the threaded hole. This is true on some of the bolts; I don't know how many. This was on the upper lateral restraint of the steam generators.

I was also told of unqualified personnel doing welding. And there were also craft people signing off on the maintenance cards instead of QC inspectors; in other words, forging the QC inspector's name. There was'also improper documentation of maintenance cards. For instance, there was a shaft taken out of a motor in unit 2 and placed in unit 1 and for three months the maintenance card hold point which was called for the shaft to be rotated was signed off by verification by QC when in fact there wasn't even a shaft there.

There was also some welding done on a valve with the valve seat closed. Mill-wrights realized that they should have had it open and tried to get day shift QC to go ahead and buy it off; when the day shift QC would not buy it off, they opened it up and have the night shift QC inspector to buy it off.

I also have a notarized statement from a person about when he was core boring, he found some areas in the concrete where the concrete was so powdery that it wouldn't even form a core.

Also, in the stainless steel fuel pool, north and south, there is some stainless steel spray pipe. The pipe came with holes drilled in them and someone decided to drill the holes larger. The pipe department drilled the i

holes larger; now the pipe is full of drill shavings and cutting oil. (At least it was when I lef t.) As far as I know, it was done without QC being present. I don't think it called for any paperwork, but the shavings should be cleaned out so they won't get into the fuel cell racks and cause any problems l like jamming. I called tnis to my superintendent's attention; he said there l

would be deminerali.ted water in there. What's the use of putting demineralized l

' That's all I can think of right now. There's so much that went on that I keep remembering things. When I do, I'll be in touch with CASE and can be contacted through them if the NRC wants to get in touch with me. I'll be i
