SUMMARY

Aitotalfof 44 pipe rupture --restraints are being fabricated by Bergen-Paterson Pipe - Support - Corpo ration for BVPS-2.

In this ins t ance, -.

Bergen-Paterson is a subvendor to Applied Engineering Company.

On1May 5,

1983, a'stop work order was ' issued to Bergen-Paterson by ' Applied Engineering Company for the reasons listed below:

All' 16 were.. reject ed because

i.

The ' clevis design requires that all welds 'on the carbon steel

. clevises be = full penetration welds.

None of the 16 units had complete penetration welds.

ii. A number of welds on the subject ~ clevises _ exhibited lack of.

fusion, _ overlap or cold roll, porosity, undercut and insuf fi-

. cient weld reinforcement.

IMMEDIATE ACTION TAKEN As ~ not ed : in S ect ion 1, 'a s to p work o rde r was issued to Bergen-Paterson on May 5,1983, for the 44 pipe rupture : restraints for BVPS-2.

The clevis ' components of-the pipe ru ptur e restraints were not '

The welds also exhibited lack of 2 fusion,_ overlap - or ' cold' roll, po ros ity,

' undercut, and insufficient reinforcement.

B. L Nondestructive examinat ions were not pe rfo rmed in accordance with approved. procedures.

Acceptable. documentation of materials, fabrication, and ins pect ion was not provided.

4.

ANALYSIS OF SAFETY IMPLICATION Most of the pipe rupture restraints supplied by Bergen-Paterson are.to be placed ins ide the cont ainment ; several will also be located in the service building.

In the case of pipe rupture, the failure of any one pipe ru ptur e -

restraint. to -perform its intended function could' cause a propagation of

- b reaks and cause the cont ainment to.

lose its int egri ty or. simply prevent the plant from attaining safe shutdown.

Even though none of the af fect ed eq uipment has. been shipped to Beaver-Valley (BV-2), the item is de emed pot ent ially repo rt ab le due to its possibility for BV-2 af fect.

5.

CORRECTIVE ACTION TO REMEDY DEFICIENCY The following correct ive actions are being implemented to remedy the deficiency and prevent its recurrence:

A.

The unaccept ab le clevis part s have been sc rapped aryl will be replaced by newly-fabricated components B.

The n'ew clevis part s wilI be required to satisfy alI conditions of the original specification.

-C. -Retraining and requalification of all Berge n-Pa terson ins pe cto rs has been accon plished.

-D.. Nondes truct ive ~ examination report s wi11 be completed and sign ed off on the same day that the ' inspection 'is performed.

The new clevises are. scheduled to be completed by November - 1984.
