SUBJECT:

TRIP REPORT - SITE VISIT TO OBTAIN ADDITIONAL INFORMATION REGARDING RECURRING MAIN TRANSFORMER FAILURES (MTFs) AT THE NORTH ANNA POWER STATION (NAPS) ;

On July 14, 1983, the NRC and its consultant (Lawrence Livemore National Laboratory - LLNL) met at' NAPS regarding the subject noted.

General Meeting Highlights' '

O LLNL pmsented a status' update on the various tasks identified in the NRC statement of, work.

O LLNL presented a short summary on the present conclusions of its Probabilistic Risk Assessment (PRA) evaluation of the generator;and transformer protection system.

For the PRA analysis, it has been detemined that the increase in frequency of MTFs and plant nactor trip does not degrade.the plant protection system. Also, no modi-fications are warranted for the NAPS MTF protection system.

O VEPCO reported that the' investigation of the seventh failure (December 5,1982) is. complete (report to follow).

O A question / answer period was held where NRC_ requested additional information from VEPC0 regarding the MTFs.

O VEPC0 will contact GE and McGraw Edison representatives to help assist in obtaining operating and maintenance instructions for MTs.

Site Visit t

0 Pictures were taken to provide LLNL orientation of the plant physical layout to assist in the Failure Mode and Effects Analysis (FMEA) for MT fires, missiles and oil-spills.

.2-0 A tour of the turbine building and control room location was conducted with respect to potential MT missiles.

O A tour was made of the general area where reserve station service overhead lines have been relocated since the July 3,1981 MT fires.

0 A Status update was made by VEPC0 on any possible transformer bay modifications.

Conclusions:==

A formal NRC request for information to VEPC0 will be prepared based on the site visit and meet.ing discussions. A preliminary assessment following the tour determined that MTF missiles could have only insigni-ficant effects on LLNL's PRA and FEMA reports.. Oil spills and fins

will be included and expanded on in the LLNL mports.
