Dear Mr. Collins:

This letter is in response to the Notice of Violation received as a result of inspections conducted at Fort St. Vrain during the period May 1-31, 1983. The following response to the items contained in the Notice of Violation is hereby submitted:

Technical Specification 7.4.a,

" Procedures, Administrative Controls," states, in part, that, ".

written procedures shall be established, implemented, and maintained...."

Preventive Maintenance Procedure PME 21, " Preventive Maintenance on Emergency Diesel Generator Breakers,"

Issue 4, dated March 16, 1983, states, under Section 5:

"5.1 Install breaker in cubicle to test position and test close and open frem the Control Room."

• %w Administrative Procedure P-1, " Plant Operations," Issue 9, dated April 27, 1983, states, in part, that, "4.4.1 A Station Log is maintained for each day with all entries made on a 24 hour2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> clock basis.

The log assures communication of information necessary to maintain continuity between shift changes of operating personnel.

The Station Log serves as the-official record of plant operations and conditions.

It consists of:"

. clearance card as surrendering for testing and gives it to the Shift Supervisor who notifies-the-Reactor Operator and instructs him on required actions."

Contrary._. to the'above, on May 20, 1983, at approximately 1:00 p.m.'MDT, the breaker was returned to its normal position and remotely closed.onto a powered bus with the diesel generator set secured; the operator. logs did not contain an entry ~of.-this' event which resulted in the

-inoperability of a major safety-related component; the 1B emergency diesel ' generator breaker clearance was not controlled as required resulting in operator error; ~and

the clearance.was returned.to the reactor operator apparently without the Shift Supervisor's knowledge.

This is a Severity-Level IV Violation.

(1) The corrective steps -which-have been taken and the results achieved:

A meeting with all involved personnel was conducted and a formal report of the occurrence was written..This report describes-the sequence of events and the procedural / personnel actions that

-should have taken place to prevent the occurrence.

The. test. procedure has been rewritten to more clearly identify the testing requirements for the breaker involved.

The requirement that clearances are to be returned to the Shift Supervisor has been reiterated in both the report and verbally.

' Finally,- the Control Room 0perator-Schedule has been temporarily revised to include three licensed operators in the Control Room.

The most senior licensed operator has been charged with the responsibility of assuring proper test performance, and with training less experienced. operators in this area.

. '(2) Corrective steps which will be taken. to. avoid further violations:

The formal report of the cccurrence will be made required reading material as distributed by the Operating Information Assessment Group. No further future actions are anticipated.

(3).The date when full compliance will-be achieved:

The review of the report by all personnel involved in clearances 1

or testing will be completed by August 20, 1983.
