DUQUESNE LIGHT COMPANY Beaver Valley Power Station Unit No.1 Reply to Notice of Violation Inspection 83-08 Letter dated May 23, 1983 VIOLATION (Severity Level IV; Supplement I)
Description of Violation (83-08-02)
Technical Specification Surveillance Requirement 4.5.2.b, Emergency Core Cooling Systems, requires that each testable power operated valve in the flow path be fully cycled at least once per 31 days to demonstrate operability.
Contrary to the above, as of April 22,1983, M0V-SI862 A and B (the low head safety injection pump suction valves from the Refueling Water Storage Tank), which are testable power operated valves in an Emergency Core Cooling System flow path, were not being cycled on a 31 day frequency to demonstrate operability.
Discussion khile these valves were not being cycled on a 31 day frequency as required by the Technical Specifications, it should be noted that these valves were being cycled on a quarterly basis as part of the inservice inspection program.
Corrective Action Taken The requirement to cycle MOV-SI 862A and B on a 31-day frequency was added to OSTs 1.11.6 and 1.11.7 via Operating Manual Change Notices83-143 and 83-144.
Action Taken to Prevent Recurrence A check was made to verify all testable power operated valves in the ECCS flowpath are cycled on a 31-day frequency. Valves in this flow-path not cycled by a nonthly OST are those that have been determined to be untestable at power. These valves are cycled in OST 1.1.10,
" Cold Shutdown Valve Exercise Test".
Date on Which Full Compliance Will Be Achieved Full compliance for this item has been achieved at this time.
Additionally, Duquesne Light is developing a program to review pro-cedures that perform Technical Specification surveillance require-ments to ensure that each surveillance requirement is included in the appropriate procedure. This long-tem action is being tracked as un-resolved item 83-07-04.
