j ment spray pumps, the operator moticed sn increase 'in the containmer.: sump level I approzin.ately ten minutes after the Icv pressurs safety inje::fon pump had been I started. Appr x1=.a:ely 3300 gallons of water.had been pumped to tha contai= ment. About one minute later the ventils:fon isolatten actuation sigval vez received. At this cine the operator realized he had failed to follow the surveillance pro-sedures and had lef: the discharge valve of the low head safety injection pump open. j, Es inmediately secured the pump. .t The Reactor Ceolcat System was checked for leakage and contair. ment entry was e.sde approalmately one hour later. Inspiction revealed that a discharge from the cor.tainment spray nozzles had occurred. A few minutes later power raduction vaa i started. A second, contatunent entry was made about en hour later, af:er contai:enent j air samp1:s confirmed that a full face mask would prodde adequa:e respiratory pro-te tien for the levt! of radioactivity in the building. A detailed inspection revealed no serious deficiencies and no electrical grounds; tha power reduction j ves.termi=.ated at a power level of 837.. Although the operator had not followed the procedure and the discharge valve j was open, the contairs.ent spray header isolation valve (BCV-345) e.nd the low pressure ~ safety injection to :nntainment spray hsader cross-connect valve (ECV-335) should t have prev 3ctoil the event. 'the ele::ric/pneumati: converter en ECV-345 had failed ,k and both red and green, position indicatics lights were on, indica:ing the valve was partially cpen. Prior to the event the auxiliary Building Equipment Operator had j taken local control of the valve in an attenpt to completely r!ose the valve. After about 1/2 inch of stem travel, the operator removed ths valw pin and the valve 4 went back to its previous position as demanded by the valvs positioner. The third valve (ECV-335) in the incident had a leakass problen that had been previously identified but no corrective action had been taken. The pneumatic relay on valve KC7-345 was replaced and valve ECV-335 repaired $g Valve ECV-31.4 and RCV-345 sre now required to be placad in the tsst mode prior io-operating the low pressure safety injection pump or contain apray pump for tasting. j This mode along with verifiestion of an snmncistor vill ensure that both of thess l valves are in the fully clesed position prior to pump operation.
