L Attached is MacMillan's instruction to me to review our performance with regard to such experiences as the Davis.Besse incident of Septesber 24. 1977, and to come up with conclusions and recommendations t

for actions taken or to be taken by 1 Septester 1979. In view of the several actions that have been underway, going back to the Availability 3 ;

Program of January (!tes 4.7. Communications Circuit to the Site). I believe that action shculd be completed on this item by 15 August.

t l

As discussed separately with the action addressees of this memo.

I propose we proceed to finish up the action on each segment of this problem area with assignments of responsibility as recognized I

d In treating each of these subject areas, I would appreciate a draft me crandum suitable for including in a total report which rakes known where the procecures.

policy and practice now in effect are adequate, acceptable; changes in p.::ced.;res and practices which have been defined, whether iglecented or not; and/or r.ew changes that are proposed that have not yet been agreed to and are not yet

[

implemented.

I Above and beyond having each of these segrents treated to display and cc:>-

municate our congrwhensive program, I believe that additional steps will be needed which should include at least the follcwing:

o A green bullet should be prepared connunicating the General Manager's message to all esployees of the need for their initiative and follow-through to assure that responsible action is taken for problems in perfonnance of our product.

r.

o 'de should be prepared to implerent intensive training sessions within a sonth of the issue of upgr.aded procedures to reach r

enough of the leaders of the Division to assure that the new practices will go into effect feraediately.

[.

o Management reviews should be conducted.per my discussions with g

DeCar11 such as to confirm that all segments of the Division and I

all elements in the process of handling these proble:s are i

being effectively carried put. Such reviews should te done so I

that each year every element of the process and every participating organizational unit is checked. Findings and reccerendations fer

[.

corrective actions should be the subject of.a report for operating f l plants to Kosiba, and for NSS plants to Oeddens.

g One additional action ! believe to be appropriate in response to MacMillan's T

charter is separate frem the independent steps of a positive policy and practice for handling operating experience. I balleve that one of the f

elements of the Davis-Besse expertence is that rather than using established

- +

channels as a manner of. normal practice to raise concerns in such a way that they could be tracked and followed up, the participants followed a scre hat

[

nor-41 practice of the Division of writing a merorandum. In this instance and in several other prcbles areas shat we have discussed, these cercrande ficat

[

around the Division and are answered in whole or in part such that it is very l

easy for u.e participants to believe that action was taken to resolve the

[

prc31em, when in fact it has merely been discussed.' Therefore, I believe r

(

that actions should be taken by each Cepartment Manager to institute those ceasures necessary to provide for supervision of mercranda. By su::ervisten l

1 rean the gamut of practices which include such steps as defining who is allcwed to sign reens that go outside the Organizacioral unit he works in.

I or outside the Cepartment, or outside the Olvisien; and also the practice of l

providing for review of aerorands with prc::er coaching to assure that they are fccused on specific action parties ar.d are felicwed up in due course.

I would appreciate your coment cn hcw concerted effort can best be effac ec in this area. My present belief is that we would achieve she cost results by having this an independent depart:ent activity until we have so.e ex.

perience with the various techniques which are available. Possibly in six months we would be in a positico to.erite guidance in this regard for the entire Division, but to do so pre..aturely, in ry judg ent, could j

result in red-tape rather than effective supervision.

l

.n all of the above, there is an eiertent of providing for proper funding.

The discussions of the iterns should ackr.owledge funding that has been provided, or should identify the funding and control of funding.and the accountability for the effective use of such funding that gives the best cnance of successful and responsible activity in this 2rea. Many people in the Division believe that the availanility of funding has cost the i

Civision many manhours of discussion which would be better utilized in actually solving the problem.

I suggest that each of the draft reports for the ses ents as assigned be prepared by the 1st of August, and also that we plan to get together and discuss these drafts on the 3rd of August. Virginia is re yested to set
