SUBJECT:
EXPIRED LICENSE, ROBERT PACKER HOSPITAL, SAYRE, PENNSYLVANIA On March 18, 1983, a licensee representative, Mr. E. H. Crosby, was contacted and informed that License No. SNM-1758 had expired on August 31, 1982, and that no renewal application had been received.
The licensee representative explained that they never received or possessed licensed material and wished to have the license terminated. This was confirmed in a letter from the licensee dated April 18, 1983.
Based on a review of the information supplied by the licensee and a review of the type and for:n of material authorized by the licensee, this license may be retired.
Based on the above information, we recommend that License No. SNM-1758 be retired.
Please notify us when your action is complete and the status has beu, changed to "4" so we may remove the file from our shelves.
