INSPECTION REPORT NO. 50-483/8 -14 This reply is in response to your letter of March 29, 1983 which transmitted the report of the inspection conducted at Callaway Plant, Unit 1 during the period of November 1, 1982 to November 5, 1982.
Our response to the item of noncompliance is presented below.
None of the material in the inspection report or in this response is considered proprietary by Union Electric Company.
(50-483/82-14-Ola) SEVERITY LEVEL IV VIOLATION Paragraph C.3 of Enclosure B to Union Electric Company letter number ULNRC-271, dated July 5, 1978 states, " Receipt inspection or performance testing will verify that material, equipment and services pertaining to the site related portions of the fire protection system conform to procurement documents",
Contrar;r to the above, missile resistant door number 36042 has not been tested in accordance with Union Electric Company Procurement l
Specification 10466-A-076, Revision 2, Paragraph 5.1.2(e), in that no acceptable documentation was available for the inspector's review to indicate that this door met the test requirements of ASTM E-152 for a Class A (3 hour3.472222e-5 days <br />8.333333e-4 hours <br />4.960317e-6 weeks <br />1.1415e-6 months <br /> fire rating) door, in that the manufacturer stated that no tests had been performed on this type door.
No permanently affixed fire-rating label was observed on the door by the inspectors.

Bechtel, the Lead A/E, is performing an engineering comparison between the existing door, 36042, and a qualified door of lesser i

Union Electric expects to have the results of the comparison on July 29, 1983.

Little April 27, 1983 (50-483/82-14-Olb) SEVERITY LEVEL IV VIOLATION 10 CFR 50, Appendix B, Criterion VII states in part, " Measures shall be established to assure that purchased material, equipment, and services,... conform to the procurement documents".
Contrary to the above, fire doors numbered 4, 5 and 33011 failed to meet the acceptable criteria in accordance with design package 23, Drawing No. 8600-X-8846, Revision 3, in that no documentation was available nor were labels affixed for the inspectors review to indicate that these doors conformed to U.L.
requirements as required by paragraph 5.8 of Specification No. 4645-23A, Revision 1.
Corrective Action Taken And The Results Achieved:
1.
A Nonconformance Report,No. 2NN-3804-C was written on November 22, 1982, covering doors 4/23 and 5/23.
DIC is presently review-ing the A/E approved disposition of the NCR to determine the most feasible and er:peditious method for correcting this deficiency.
DIC Engineering has performed a complete walkdown of all outside area buildings to determine whether the doors installed meet design and label requirements.
This walkdown was prior to any Quality Control inspections as an in-process status report for Room and Area turnover.
One other deficient opening was identified.and was documented on Deficiency Report No. 2ND-2034-C which is enroute for disposition / approval.
The vendor was notified and is expected to complete his investiga-tion by June 1, 1983.

The Fuel Building walkdown and Quality Control final inspection werecompleted prior to Fuel Building turnover.
No deficiencies were noted as to improper or missing fire rating labels.
A walkdown of all other fire rated openings in the Power Block area was completed January 3, 1983.
This walkdown identified 26 openings having improper labels or no labels at all.
Areas covered in this walkdown are for statusing Room and Area reports; final Quality Control inspections will be done at a later date.
The vendor was supplied with this list of openings to check factory records and inspect doors and frames that have no labels or improper labels to determine whether they were manu-factured to meet UL requirements for the label class required.
On February 4, 1983, Deficiency Report No. 2ND-1719-C was written on these 26 openings, which included opening No. 33011.
As of April 11, 21 openings have been properly tagged with UL labeln and 5 openings remain under investigation by the vendor, including opening No. 33011.
The vendor stated that this action is expected to be completed by June 1, 1983.

Little April 27, 1983 Corrective Action To Be Taken To Avoid Further Noncompliance:
To prevent recurrence, material receiving instructions require Quality Inspections on all new fire rated doors and frames prior to acceptance on site.
The Date When Full Compliance Will Be Achieved:
1.
DIC will complete the review and evaluation of the disposition to NCR 2NN-3804-C on April 29, 1983.
The expected completion date for DR 2ND-2034-C is July 1, 1983.
If new material is to be purchased, completion will depend on availability of material.
2.
DIC' expects the completion date to be July 1,.1983; however, this is contingent on the vendor and the availability of materials.
Please note that criteria for doors 4 and 5 are contained in Design Package 23, Drawing No. 8600-X-88446.
Criteria for door No. 33011 arecontained in Specification 10466-A070, Revision 6, Sections 6.1.6 and 6.2.6.
The appropriate drawing numbers are Door Schedule 10466-A-0908, Revision 12, and Floor Plan at Elevation 2000, 10466-A-0325, Revision 13.
