NONCONFORMANCES:

Contrary to Criterion V of Appendix B to 10 CFR Part 50 and paragraph 5.7.2 of the Quality Assurance Manual, four lots (115-1, 128-1, 114-1, and 113-1) of control element assemblies were found in the fabrication process for which all of the applicable release numbers had not been recorded on the travelers; e.g., the release number for the poison pellets and compression sleeve used for lot 115-1 had not been entered and the traveler for lot 128-1 was missing the release number for the poison pellets and spring that were used.

Contrary to Criterion V of Appendix B to 10 CFR Part 50 and paragraph E.2 of Operation Sheet No.1242, the NRC inspector observed examples where inspection operations on travelers had been initialed without the prior initialing (acceptance) of all applicable charac-teristics on the relevant Inspection Record Forms.
The travelers for lots 115-1 and 128-1 control element assemblies had operation 525 initialed which pertained to inspection characteristics 4, 8, 14-19, and 40. The relevant Inspection Record Forms did not, however, have characteristics 14-19 for lot 115-1 and characteristics 19 and 40 for lot 128-1 initialed.

STATUS OF PREVIOUS INSPECTION FINDINGS:
1.
(Closed) Nonconformance (Item A, 82-01):
Duplicate boat numbers and four missing tray numbers were found in a lot of pellets being processed.
CE has initiated an overcheck of pellet records by Quality Control Engineering.
A recent lot of pellets was reviewed and all tray numbers and boat numbers were found to be accounted for on the records.
No further problem was found in this area.

Work was being performed to a revision of a specification which was not included in the Manufacturing Order (MO).
The specification in question has now been added to the Manufacturing Order and Quality Control Engineering has been instructed to review M0s in this regard. An M0 for a current project was reviewed without identification of any further problem.

(Closed) Nonconformance (Item C, 82-01): A review of 10 " Personnel Qualification" records found records which had been issued in 1980 and 1981 that stated 2 persons had been qualified to a revision of SNT-TC-1A other than that specified in the QA manual.
SNT-TC-1A (1975) was specified in the QA manual, but records indicated qualification to SNT-TC-1A (1969) or SNT-TC-1A (1980).
CE has reviewed and revised the certification records for NDE personnel. The personnel certification records were reviewed by the NRC inspector and no additional problems were noted.

OTHER FINDINGS OR COMMENTS:
Miscellaneous Components - The fabrication of control element assemblies (B G rods) was inspected and a review made of the applicable drawings, g
specification, M0s, travelers, and procedures.
The use of qualified per-sonnel, accepted visual standards, and calibrated equipment was verified and an inspection made in regard to compliance with welding requirements.
A nonconformance was identified (see B.1 above) regarding use.of com-ponents without recording all release numbers on the travelers. An additional nonconformance was identified pertaining to signoff of inspection operations on travelers prior to initialing (acceptance) of all applicable characteristics on the relevant Inspection Record Form (see B.2 above).
CE personnel took immediate corrective action on both of these problems and completed the missing information on the travelers in question.
