A special radiological controls inspection was conducted by Messrs.

P. O'Connell and D. Chawaga of this office during the period of March

14-15, 1990.

The purpose of this inspection was to review the circumstances,

!

dose assessments. evaluations, and corrective actions associated with the

sodium 24 (Na.24) personnel contamination incident which occurred on March 8

!

1990. ThefindingsoftheinspectionwerediscussedwithyouonMarch15,lh90.

involve failure to properly instruct personnel working with a high specific

activity liquid solution and failure of personnel to properly prepare and adhere

to procedures involving personnel radiation protection during flow tests.

Because the findings of this inspection indicate significant weaknesses in the

!

planning and control of non-routine radiololically significant work activities,

i

including the control of contamination, we

tave scheduled an enforcement

!

conference with you to discuss our findings and your corrective actions. We are

particularly concerned that this lack of control resulted in an unplanned

!

occupational radiation exposure to a worker possibly in excess of NRC limits.

i

This issue is being categorized as unresolved pending your final dose assessment

i

and the assigning of a dose of record,

The enforcement conference has been scheduled for 10:00 a.m. on April 9 1990
