ENCLOSURE 1 NOTICE OF VIOLATION i

South Carolina Electric & Gas Docket No. 50-395 V. C. Summer Nuclear Station License No. NPF-12 During the Nuc1 car Regulatory Comission (NRC) inspection conducted on February 1 - 28, 1990, violations of NRC requirements were identified.

In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2 Appendix C (1989), the violations are listed below:

A.

Technical Specification 6.8.1.a requires that written procedures be established, implemented, and maintained for activities referenced in Appendix A of Regulatory Guide 1.33, Rev. 2. February 1978.

Station Administrative Procedure, Conduct of Maintenance, SAP-300, Rev. 4 requires that the group performing the maintenance activity be responsible i

for all programmatic controls including notification of operations and for ensuring that Health Physics concurs with the radiation work permit used for the particular work activity.

Station Administrative Procedure, Application, Scheduling and Handling of Maintenance Activities, SAP-603, Rev. 6, requires that the organization performing the work be responsible for notifying operations prior to job start.

Mechanical Maintenance Procedure, Remote and/or Contact Changeout of Contaminated Filters, MMP-225-003, Rev. 3, requires that a radiation work permit be obtained if required for the work request, and also requires that the system be isolated, tagged, and drained prior to doing the work. Health Physics Procedure, Use of The Radiation kork Permit and Standing Radiation Work Permit, HPP-151 Rev. 5, requires that the job supervisor and work crew review the RWP prior to beginning work, and requires that the job supervisor ensure that special instructions are followed.

Contrary to the above, on February 1,1990, a mechanical maintenance crew and the health physics crew failed to follow these procedures in that they proceeded to perform the maintenance work on the reactor coolant filter without notifying the shift supervisor for operation, without tagging the system, without isolating and draining the system, and entered the radiation control area without a proper radiation work permit.

This resulted in a spill of chemical volume and control system water of about 250 gallons and radiation exposure of 215 mrem during decontamination.

B.

Systems Operating Procedure, Main Steam System, S0P 201, Rev. 8,, Page 37, requires that instrument air root supply valve IFV 2030-AVI-MS be maintained in the open position. The Backup Air Supply Check Valve Test For Emergency Feedwater Valves Surveillance Test Procedure, 102.007, Rev. 2, Attachment 2, page 2, requires that the abcve valve be returned to the open position after the completion of the above surveillance test procedure.

L Contrary to the above, the turbine driven emergency feedwater pump was found running at approximately 12:30 p.m., on February 20, 1990, with valve IFV 2030-AVI-MS in the closed position.

L These violations have been evaluated in the aggregate as a Severity LevelIV(Supplement 1).

Sumwr W. lear Station, vmin 30 days of the date of the letter transmitting this Notice.

This reply swid be clearly marked as a

" Reply to a Notice of Violation" and should include (for each violation):

(1) admission or denial of the violation, (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the results achieved. (4) the corrective steps which will be taken to avoid further violations, and (5) the dhte when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.

If an adequate reply is not received within the time specified in this Notice, an order may be issued to show cause why the license should not be modified, suspended, or revoked or why such other action as may be proper should not be taken.
