This license is subject to an annual fee for sources of radioactivo ' material authorized to be possessed at any one time as specified in Item 8 of this p
license.
The - annual fee for this license is required by and computed in accordance with Sections 30230-30232 of the California Radiation Control Regulations and is also subject to an annual cost-of-living adjustment pursuant to Section 113 of the California Health and Safety Code.

Except as specifically provided otherwise by this license, the licensee shall possess and use radioactive material described in Items 6, 7,
and 8 of this license in accordance with statements, representations, and procedures contained in che documents listed below.
The Department's regulations shall govern unless the statements, representations, and procedures in the licensee's application and correspondence are more restrictive than the regulations, j
(a) The application with - attachments dated July 29, 1989, signed by Isaac Bertrand and as modified by letters with attachments September 4, 1989 and October 11, 1989 both signed by Isaac Bertrand.
(b) The letter with attachment' dated November 2, 1989 signed by Isaac Bertrand regarding procedure for source exchange using SPEC Hodel C-1 source exchanger.
14.
(a) The radiation safety officer in this program shall be Isaac Bertrand.
(b) The alternate radiation safety officer shall be Spencer Bechtel.
15.
Sealed sources possessed under this license shall be tested for leakage and/or contamination as required by Section 30275(c) of the California Radiation Control Regulations.

- 16. -The following individuals are authorized to collect wipe test samples of sealed sources possessed under this license using leak test kits acceptable to the California Department of Health Services, w
I (a) The radiation safety officer.
(b) Qualified radiographer designated by the radiation safety officer.
17.
Analytical tests for leakage and/or - contamination of sealed sources shall be f
performed only by persons specifically authorized to perform that service.
18.
Records of leak test-results shall be kept in units of microcuries and l
maintained for inspection.
Records may be disposed of following Department inspection.
Any leak test revealing the presence of 0.005 microcuries or more of removable radioactive material shall be reported to the Department of Health Services, Radiologic Health Branch, 744 P Street, P.O.

CA 94234-7320, within five days of the test.
This report shall _ include a description of the defective source or device, the results of the test,'and the j
corrective action taken.
19.
Pursuant to California Radiation Control Regulations, the licensee.is authorized to possess up.co 999 kilograms (2,203 pounds) of natural or depleted uranium used for purposes of shielding or co111mation in radiographic exposure devices
{
listed in Item 9 of this license.

At any t~me the licensee is engaged in the performance of industrial radiography l
by authority of this license, at either a permanent or a temporary job site, he.
j shall have a current copy of each of the following documents available for inspection at the site:
(a)
California Radiation Control Regulations.
(b).This license.
(c) The licensee's operating and emergency procedures.
21.
The licensee shall not authorize any radiographer to use any model of a radiographic exposure device, related handling tool. or radiation survey instrument until the radiographer has been trained in the use of such equipment, and has demonstrated competence in its use.
For each radiographer, the licensee shall maintain the following records available for inspection:

(a) Training received by the radiographer in the use of such equipment, including dates, duration, and name of instructor.

I' (b) Each radiographic exposure device for which the. radiographer has demonstrated competence in its use, including date of demonstration of competence.
22.
(a) The licensee. shall conduct refresher training for all radiographers and radiographers' assistants at intervals: not - to exceed six months. Such training shall include, but not limited to:
(1) California Radiation Control Regulations, with emphasis on Sections 30330 30334.
(2) Subj ect matter listed in Section 30335 of the California Radiation
-Control Regulations.
(3) This license.
(4) The licensee's operating and emergency procedures, with emphasis on any changes which have been made within one year.
(5) Equipment used under this -license for performance of industrial radiography with emphasis on equipment which has been used by the licensee for less than one year.
