Sequoyah Units 1 & 2 License Nos. DPR-77, DPR-79 During the Nuclear Regulatory Commission (NRC) inspection conducted on ilanuary 22-26, 1990, a violation of NRC requirements was identified.' In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1989), the violation is 4
listed below:
10 CFR 50, Appendix B, Criterion V,
" Instructions, Procedures and Drawings," requires that activities affecting quality shell be prescribed by appropriate procedures and shall be accomplished in accordance with a
those procedures.
In addition, the TVA Nuclear Quality Assurance Plan, TVA-NQA-PLN89, Revision 0, requires that mandatory controls be established to meet the requirements of ANSI N45.2.11-1974, Quality Assurance Require-ments for the Design of Nuclear Power Plants. ANSI N45.2.11-1974 requires design basis documentation to be sufficient to trace the design back to its input and be such that it can be accessed and verified by. competent reviewers without recourse to the original designer.
Procedures NEP-3.1, Calc.alation, and NEP-3.2, Design Input, implement these requirements.
Contrary to the above, the technical justification for Engineering Change Notice L7242E, to demonstrate that an addition of 8.2 KW would have no adverse effect on emergency diesel generator loading, was not developed through a documented calculation as specified by procedures NEP-3.1 and NEP-3.2.
This is a Severity Level IV violation (Supplement I).
1 Pursuant to the provisions of 10 CFR 2.201, Tennessee Valley Authority, is hereby required to submit a written statement or explanation to the Nuclear Regulatory Commission, ATTN: - Document Control Desk, Washington, DC 20555, with a copy to the Associate Director for Special Projects, Office of Nuclear Reactor Regulation and a copy to the NRC Resident Inspector, Sequoyah, within 30 days of the date of the letter transmitting this Notice. This reply should be clearly marked as a " Reply to a Notice of Violation" and should include:
(1). admission or denial of the violation, (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the results achieved, (4) the corrective steps which will be taken to avoid further violations, and (5) the date when full compliance will be achieved.
