'Ihis appttwal is in acccedance with a anacrandum from the Director, Office of Water Resources Management. A copy is enclosed for your information.
Virginia NPEES Permit No. VA0004774 is also enclosed. In accordance with the permit, the permittee'is required to submit monitoring reports to the West omntral Regional office. '2his reporting form is included with the permit. 'Iha permittee will be responsible for obtaining additional copies of the IWdng form.
