Dear Mr. Crase:
This refers to your application dated December 6, 1989, requesting renewal of Certificate of Compliance No. 6144 for the Model No. B-2 package. Since an application was submitted at least 30 days prior to expiration, the Certificate i
of Compliance will not be deemed to have expired until the application has l
finally been determined.
The application for renewal does not include operating procedures or maintenance programs for the package.
The application should be revised to include, as a
-minimum, the following information.
t Operating procedures should be presented sequentially in the order of performance l
and such items as' inspections prior to loading, cautionary notes on package handling, limits on package contents, installation of a security seal, and instructions pertinent to receiving and unloading the package should be addressed.
l The maintenance program should describe the actions taken to ensure continued

-performance of the packaging. The program should include periodic testing, inspection, and replacement schedules, as well as criteria for replacement l
and repair of components and subsystems on an as-needed basis.
This information should be provided within 90 days from the date of this letter.
