This is in reference to your letter September 29, 1989 informing us that our Quality Assurance Program Approval for Radioactive Packages is up for renewal.

We wish to advise you that we do want to renew our Q. A. Program and are enclosing our check for the amount of $150.00 in payment of the required fee.

At this time I would like to call your attention to the following changes-that need to be made:

1) The name on our license is under X-R-I Testing a division of X-Ray l

Industries Inc. This has been changed from X-R-I Teeting Of Michigan, l

Inc.

DESCRIPTION 10CFR71 QUALITY ASSURANCE PROGRAM INDUSTRIAL BYPRODUCT MATERIAL LICENSE # 21-05472-01 1.

ORGANIZATION The final responsibility for the Quality Assurance Program for part 71 requirements rest with X-R-I Testing a division of X-Ray Indus tri e s, Inc.

Design and fabrication of radioactive material shipping packages shall meet the applicable standards and not be a part of this Quality Assurance Program.

X-R-I Testing's Quality Assurance Program is implemented using the organization described in Figure 1. attached.

a)

The Radiation Safety Officer shall be responsible for overall administration of the program, training and certification, document control, and auditing, b)

The Radiographers are responsible for handling, storing, transporting, shipping, inspection, operating status and record keeping as outlined in the Operating and Emergency Procedures for X-R-I Testing.

2.

QUALITY ASSURANCE PROGRAM The management of X-R-I Testing, establishes and implements this Quality Assurance Program. Training, prior to engagement for all Q. A. functions is required according to written procedures.

Program revisions will be made according to written procedures l

with management approval.

The' Q. A. program will ensure that all defined Q. A. procedures, engineering procedures and specific provisions of the package design approval are satisfied.

The Q. A. Program will emphasize control of the characteristics of the r

package which are critical to safety.

The Radiation Safety Officer shall assure that all radioactive material shipping packages are approved by the Nuclear Regulatory Commission.

This Shall be satisfied by a certification from.the Manufacturer when containers are obtained.

3 DOCUMENT CONTROL a)

All documents related to a specific shipping package will be-controlled through the use of established procedures.

b)

Radiographers are responsible for documents pertaining to a particular container prior to the shipment or transport of any radioactive material exposure device.

c)

The procedure as outlined in Section VIII shall be used as a L

guideline prior to tre.nsport of any material.

AUDITS a)

The management of X-R-I Testing shall perform periodic inspection and shall include the followings a.1) Proper paperwork a.2) Daily check list (Section 10, Page 1) a.3) Adherence to Section 8 (Transportation) b)

Results of audits will be maintained on file for future reference.

c)

Deficient areas will be evaluated and corrected promptly 5

HANDLING, STORING, SHIPPING, TRANSPORTING FOR USE The responsibility for handling, storing, shipping, and transporting for use, rests with the Radiographers as listed in Section 2, Page 2 of X-R-I Testing's Operating and Dnergency Procedures for safe use of Dyproduct Materials.

a)

These procedures shall be performed by Qualified Radiographers employed by X-R-I Testing b)

Handling and transporting shall not be performed until the procedures in X-R-I Testing's Operating and Emergency Procedures Manual, Section 3, page 9 have been completed.

After this has been accomplished and all' conditions satisfy Section 3, Page 9, the container can then be made ready for transport.

c)

Two (2) Qualified Radiographers will perform the operation of making the source container ready for shipment at all times, and shall include the following:

c.1) Disconnect of the drive cables prior to placement of camera into container, c.2) Survey of camera before placement into container and after container has been secured.

c.3) Proper tag on camera and related decay chart.

6.

INSPECTION, TEST AND OPERATING STATUS a)

The responsibility for inspection, test and operating status of packages for certain special form radioactive material will rest with the radiographers, b)

Adequate records indicating results of the inspection, test and operating status of such packages shall be rtaintained and kept on file.

c)

It is the responsibility of the Radiation Safety Officer to ensure that these functions are performed.

QUALITY ASSURANCE RECORDS a)

Complete records of package approvals (including references =

and drawings), inspections, test, operating logs, audit results, personnel training and qualifications and records of-shipment will be ' maintained by the Radiation Safety Records

Office under the Supervision of the Radiation Safety Officer.

b)

Radiographers and Radiographers Assistants shall have in their possession a copy of their-Operating and Emergency Procedures and certification of package approval from the Manufacturer.
