Gentlemen:

This refers to the inspection conducted by Mr. P. Harrell of this office during

the period October 12 through November 30, 1989. of activities authorized by

NRC Operating License DPR-40 for the Fort Calhoun Station, and to the

discussion of our findings with you and other members of your staff at the

conclusion of the inspection.

Areas examined during the inspection included review of onsite events. Within

this area, the inspection consisted of selective examination of procedures and

representative records, interviews with personnel, and observations by the

inspector. The inspection findings are documented in the enclosed inspection

,

report.

During this inspection, it was found that certain of your activities were in

6pparent violation of NRC requimments. These concerns involve work control

,

and postnaintenance testing problems' resulting in two component cooling water

heat exchangers being inoperable concurrently for approximately 6 days and a

,

,

procedure control issue resulting in your apparent failure to comply with

10 CFR 50.59 for determination of an unreviewed safety question involving the

'

issuance of change to a plant procedure. As discussed in our telephone

'

conversation of December 4,1989, with Mr. J. Chase, we have scheduled an

enforcement conference in our office at 10 a.m. on January 5,1990. At this

meeting, we will discuss your understanding of the apparent violations in this

report, the reason for their occurrence, and your corrective actions.
