DPR-50 and DPR-73 Docket No. 50-289 and 50-320 Response to Combined inspection Report 50-289/89-22 and 50-320/89-10 The subject Inspection Report identified one item of noncompliance concerning the failure to disqualify an armed Site Protection Officer who did not meet the minbum physical qualifications for visual acuity. Attached is GPU Nuclear's response to this item of noncompliance.

4410-89-L-0126 Item of Noncompliance "The NRC-approved Three Mile Island Security Personnel Training and Qualification Plan dated November 8,1985, Section 1.2.1.3, Armed Personnel Requirements, states, in part, that for each individual, distance visual acuity in each eye shall be correctable to 20/20 in the better eye and 20/40 in the otner eye with eyeglasses or contact lenses.
Contrary to the above, on October 11, 1989, during a review of medical records, an armed security officer, on duty: was identified with corrected visual acuity less than 20/40 in the weak eye.

This is a Severity Level IV violation.

GPU Nuclear Response GPU Nuclear concurs with this item of nonenmpliance. The cause of this violation is attributed to the failure of the Medical Department to immediately notify the Security Department of the disqualification of a Site Protection Officer for armed duty.
The failure of GPU Nuclear to detect this event previously was due to the lack of accessibility to medical records by the Quality Assurance (QA) Department during performance of QA audits.
A.
Corrective Actions Taken The Site Protection Officer referred to in the item of noncompliance was removed from armed duty upon identification of this event. A QA audit c
(i.e., S-TMI-89-20 dated 12/5/89) was subsequently performed of the medical records of all armed Site Protection Officers to determine compliance with the medical qualification requirements of the NRC-approved Training and o
l Qualification Plan.
As noted in the subject Inspection Report, this audit L
identified an additional armed Site Protection Officer who failed to meet I
the minimum visual acuity requirements. This individual was also promptly l-relieved of armed response duties.
In addition to Site Protection personnel, the referenced QA audit also reviewed the medical records of licensed operators, crane operators, and individucis who utilize respiratory protection devices. This audit identified four (4) findings, which are currently being corrected.
B.
Corrective Actions Which Will Be Taken to Avoid Further Violations When an individual is disqualified for medical reasons, the Medical Department will issue a Restricted Duty Form (Exhibit 1) which will l
identify the specific work restrictions attributable to any failed physical

requirement.
In the case of Site' Protection personnel, a copy of this form L
is immediately issued (i.e., same working day) to the Security Department and the Site Protection Officer. Additionally, the Medical Department notifies the Human Resources Department when an individual has failed a i
medical exam. The Human Resources Department then notifies the individual's supervisor, per Procedure 1000-ADM-2624.01.
Future QA audits will include direct examination of medical records pertaining to the minimum qualifications mandated by regulatory requirements considering the confidentiality of those medical records. GPU Nuclear believes these corrective actions should minimize the potential for similar violations.
C.

corrective actions will be fully implemented.
