Appendix A NOTICE OF DEVIATION Ohio State University Docket No. 50-150 As a result of the inspection conducted on tugust 17-21, 1981, it appears that certain of your activities deviate from commitments made to the NRC by the Ohio State University.
Deviation The Hazards Suramary Report (HSR) and revisions to it, approved by the Reactor Operations Committee on July 22, 1980, states," 2.2.1-Written procedures, reviewed and approved by a licensed Senior Resctor Operator, shall be in effect and followed."
Contrary to the above, there were several instances where HSR commit-ments and written procedures had not been followed as listed below:
1.
Failure to keep the Operator Experience Log current, Procedure AP-10.
2.
Failure to control the RY-4 relay removal and jumper installation, Procedure 1-6.
3.
Failure of the ROC to meet on a quarterly frequency, HSR 2 1.3.
4.
Failure to document the review and knowledge of new and revised procedures, Procedure AP-7.
5.
Failure on two cecisions to meet the monthly particulate sample frequency, Procedure RS-4.
6.
Fai.are to have complete radiation safety training records, Procedure RS li.
7.
Failure to have complete security training records, Procedure SP-4.
