Docket No. 99900524/81-03 NOTICE OF NONCONFORMANCE Based on-the results of an NRC inspection conducted on August 4-7, 1981, it appears that certain of your activities were not conducted in accordance with NRC requirements.

Criterion V of Appendix B to 10 CFR Part 50 states:

" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.

Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for detennining that important activities have been satisfactorily accomplished."

Gibbs and Hill (G&H) Comanche Peak Steam Electric Station project proce-dure PC-5 (Vendor Drawing Review Procedure) states, in part that, "...

a drawing fulfilling the requirements of the specification and satisfying the needs-of in-house design are (sic) considered approved... the drawings, prior to their return to the vendor, are stamped as' follows: APPROVED...

Contrary to the above, vendor drawings, depicting valves with 300 PSIG rated flanges to be supplied by Westinghouse in accordance with their Equipment Specification for use in certain portions of the 600 PSIG rated safety-related safety injection and residual heat removal systems, were approved by G&H even though the valve flanges did not satisfy the needs of the G&H in-house design requirement for these 600 PSIG rated systems.
