This refers to ~ the routine safety inspection conducted by Mr. J. R. Mu11auer of this office on September 17, 1981, of activities authorized by NRC Byproduct Material Licenses No. 13-02128-02 and No. 13-02128-03 and to the discussion of Jour findings with~ Ralph K. Yacko and other memb es of your staff at the conclusion of the inspection.

The inspection was an examination of activities conducted under your license

-as they relate to radiation safety'and to_ compliance with the Commission's rules and regulations and with the' conditions of your license. The inspec-

' tion consisted of a selective examination of procedures and representative records, observations, independent measurements, and interviews with personnel.

In addition to th'e above-areas, the inspector examined actions described in i

letters _ dated January 6,' 1978 and December 20, 1977, regarding corrective

action' taken on apparent items of noncompliance found during our November 22 and 23, 1977, inspection of License No. 13-02128-02 and inspection of License No. 13-02128-03. We have no further questions regarding these matters.

' This inspection also included a measurement of the output of-your teletherapy equipment.~ Our measurement was made using a Victoreen 'Model 570 R-Meter with Model'621 chamber, calibrated by the Na tional Bureau of Standards Laboratory.

' The source-to-chamber distance was 80 centimeters and the field size used was ten by ten centimeters. After applying standard correction factors for tem-perature, pressure, attenuation, inverse square, backscatter, time error,

roentgen-to-rad conversion, our measurement of the output of your teletherapy equipment _ was 178.7 rads per minute. The most recent monthly spot check measurement of your teletherapy equipment, conducted on August 31, 1981, extrapolated for 17. days decay reported a measured output for the same

. exposure parameters of 186.9 rads per minute.

~ Based on the close agreement in measured output values, wA feel the actual output of your. teletherapy equipment to be within established guidelines of 15 percent.

-During this inspection, no items of noncompliance were identified for License No. 13-02128-02; however, for License No. 13-02128-03 certain.of your activi-

~. ties appeared to be in noncompliance with NRC requirements,-as described in the enclosed Appendix A.
