NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on June 15-19, 1981, it appears that certain of your activities were not conduc.ted in accordance l

with NRC requirements as indicated below:

Criterion V of Appendix B to 10 CFR Part 50 states:

" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accom-l plished in accordance with these instructions, procedures, or drawings.

j Instructions, procedures, or drawings shall include appropriate quantita-

tive or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."

Nonconformances with these requirements are as follows:

A.

The Product Assurance Manual, June 12, 1981, Section 1.0, states that the Westinghouse Instrument Department (WID) will comply with the document l

Quality System Requirements, QCS-1.

Quality System Requirements, Section 4.4, requires design verification to be performed by either design review, alternate calculations, or testing.

Contrary to tne above, the WID Engineering Procedure 1R that addresses design change verification, and permits a method of design verification by engineering extrapolation, which is being used, but is not described in QCS-1.

Quality Assurance Procedure 14.1, Revision A, Section 3.1.1.3.11, states in part:

(On Inspection Control Tags) " Enter latest revision letter /

number...." (of wire designs) 1 Section 3.2.1.1.1 states: "The signatures of all operators who perform I

initial operation, rework, or RN installation shall be entered on the tag,"

also Section 3.3.1.8 states in part: "When required, draw line through item to signify voiding and enter stamp and date it.

Contrary to the above:

(1) The latest revision of the six wire diagrar 1 not entered 0.) the Inspection Control Tag for a Solid State Protection System.

The system, 1058E95, was given a final inspection on June 18, 1981, to the out of date revisions.

The latest revisions of the drawing had been issued on June 15, 1981.

2 (2) Several cases of the absence of initials and/or dates wrere observed.

For example, a PC board S/N 0019, 1065E47, had been pre.iented for final inspection, although RN-C-1030, a Revision Notice, was not initialed as installed on the Control Tag.

On the same laspection Control Tag a void to the first entry to the " Remarks or Discrepancy" column had not been dated.

Engineering Procedures, " Changes by RN or REA," 1M, Revision D, and

" Review of Engineering Drawings and Test Specifications," 1L, Revision E, require all changes to drawings to be documented on either a Revision Notice (RN) or Request for Engineering / Shop Action (REA).

Contrary to the above, a change was found to page 9 of drawing 2388A52 at a work station where the drawing was in use which was not documented with an RN or REA.
