Appendix A NOTICE OF VIOLATION Memorial Hospital License No. 12-18853-01 As a result of the inspection conducted on September 6, 1981, and in accord-ance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980), the following violations were identified:
1.
License Condition No. 16 requires that licensed material.be possessed and used in accordance with statements, representations, and procedures contained in application dated December 18, 1979.
The license application dated December 18, 1979, states that the pro-cedures described in Appendix D, Section 2 of the NRC Draft Licensing Guide, dated November 1977, shall be followed for calibration of the dose calibrator. Appendix D, Section 2 states that the daily constancy checks of the dose calibrator shall include _ a comparison between the actual value of a cesium-137 source and the daily reading of that source.
Contrary to this requirement, your dose calibrator does not have a cesium-137 setting which will allow you to perform the required comparison.
This is a Severity Level V violation (Supplement VII).

License Condition No. 16 requires that licensed material be possessed and used in accordance with statements, representations, and procedures con'tained in application dated December 18, 1979.
i The license application dated December 18, 1919, states that the pro-cedures described in Appendix F of the NRC Draft License Guide, dated November 1977, shall be followed for opening packages containing radio-active material. Appendix F states G-M surveys at the surface and three feet from the surface of packages containing radioactive materials shall be performed and recorded.
Contrary to this requirement, G-M survey.= were not performed at the surface and three feet from the surface of packages containing radio-active materials received from McDonough District Hospital.
This is a Severity Level V violation (Supplement VII).
3.

November 1977, shall be followed for performing routine radiation surveys. Appendix I states that the results of weekly wipe tests shall be recorded.

This is a Severity Level VI violation (Supplement VII).
1 Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each item of noncompliance:
(1) cor-rective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation. Consideration may be given to extending your response time for good cause shown.
