(Closed) Follow Up Item (00-03) HVAC Air Extractors installed in non-safety-related systems were structurally inadequate.

Bechtel and the supplier were to investigate and determine if these extractors were to be used in safety-related systems.

The investigation identified safety-related systems in which these extractors were to be used.

The licensee, Mississippi Power and Light Company, submitted their final report to NRC/RII on July 2, 1981 (Ref. PRD 80/63).

In addition the supplier, Tutle and Bailey, notified NRC/HQ per 10 CFR Part 21 of this problem on November 14, 1980.

OTHER FINDINGS OR COMMENTS:

1.

Design Verification - Applicable Engineering Department Procedures and Engineering Department Project Instructions were examined for imple-mentation of commitments made for the design verification process on the SNUPPS project.

Documents providing evidence of design verification of the Residual Heat Removal System for this project were examined i.e.

nine drawings, three specifications, and seven calculations.

It was determined that design verification of this system had been performed in accordance with applicable procedural requirements.

Design Process Management - Section 17A.1.3 of the SNUPPS PSAR and c.pplicable Engineering Department Procedures and Engineering Project Instructions were examined to determine quality assurance program com-mitments. To verify implementation of these commitments, the following documents pertaining to the SNUPPS Safety Injection System were examined:

three design specifications, three design calculations, 13 design drawings and six miscellaneous documents.

It was determined that design process management of this system had been implemented in accordance with applicable procedural requireme'ts.

In reviewing design change control it was noted that design changes were not being incorporated into the drawings in the time frame required by procedures in every case; however, this had been recognized by Bechtel quality assurance and proper action has been taken.

Design Change Control - Grand Gulf Project Engineering Procedures Manual for proper addressing of design change control were examined.

Eleven specifications, eight drawings, ten drawing change notices, and five calculations, along with their subsequent revisions i.ere reviewed to assure that design change control commitments were implemented.

In addition, the Quality Assurance Program Project Summary Q-list, along with Design Control Checklists for three design disciplines, were reviewed for proper controlling elements.

All procedural commitments were being followed properly.

Design Corrective Action - Grand Gulf Project Engineering Procedures Manual for proper addressing of Design Corrective Action were examined.

The Project Quality Engineering Quarterly design deficiency review was examined to assure that there are subsequent reviews for possible trending of recurring design deficiencies.

Ten nonconformance reports and eight Managemect Corrective Action Reports that resulted in some form of design chan3e were examined to assure t'at proper procedural commitments were followed.

Eight field change requests / notices that interfaced with project engineering were reviewed for proper disposi-tioning content. All procedural commitments were being followed properly.

Design Interfaces - Grand Gulf Project Engineering Procedures Manual for proper addressing of internal and external design interfaces were examined.

Examples of external design interfaces between Bechtel and the NSSS for the Grand Gulf project were examined as evidence of proper design information communication. Along with this examination of these transmittals, the effected design documents were reviewed to assure that incorporation of this design information was properly completed.

Five design document transmittals along with their impacting design discipline review and comment, were reviewed as examples of Bechtel's intern &i design interface processes. All procedural commitments were being followed properly.
