Appendix A NOTICE OF VIOLATION Vermillion County Hospital License No. 13-18604-01 As a result of the inspection conducted on September 15, 1981, and in ac-cordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980),
the following violations were identified:
1.
License Condition No. 15 requires that licensed material be possessed and used in accordance with statements, representations, and procedures contained in application dated October 6, 1978.
Item No. 9 ci the above referenced application states that your survey meter will have a measuring range of 0 mR/hr to 2 R/hr.

Contrary to the above requirement, it was determined through an obser-vation of the NRC inspector that this requirement has not been met.

of measuring 0 mR/hr to 50 mR/hr.

Item No. 10 of the above referenced application states that quarterly linearity' tests will be performed by assaying the syringe containing technetium-99m at six hours, 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />, 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />, and 48 hours5.555556e-4 days <br />0.0133 hours <br />7.936508e-5 weeks <br />1.8264e-5 months <br />.
Contrary to the above requirement, it was determined through state-t ments by licensee representatives that this condition is not being met.
Specifically, a licensee representative stated lineerity tests performed since August 12, 1980 did not include a six hour, 30 hour3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />, or 48 hour5.555556e-4 days <br />0.0133 hours <br />7.936508e-5 weeks <br />1.8264e-5 months <br /> assay. Furthermore, records of these tests were not available for review.
This is a Severity Level V violation (Supplement VII).
