Dear Mr. Book:

This is in response to your letter dated Augu t 11, 1981 concerning the inspection of Humboldt Bay Unit No. 3 conducted by Mr. G. Yuhas on June 29 to July 2, 1981.

Appendix A of your letter includes the following:

Technical Specification VII B.4 states in part that:

"The liquid waste system vent monitor shall normally be in service and shall be set to alarm at 10 mr/hr above background."

Contrary to the above, on June 29, 1981, the liquid waste system vent monitor was in service with a background radiation " vel of 0.5 mr/hr and was set to alarm at 100 mr/hr.

As noted in the body of the inspection report, the instrument was serviced, and the alarm setpoint adjusted to alarm at 10 iir/hr. These immediate corrective actions were completed on June 29, 1981 and verified by the inspector on July 1, 1981.

Mr. H. E. Book September 10, 1981 Subsequently, Operational Test Procedure Nos. 13 and 13B, which involve calibration and response checks of this monitor, have been revised to require that the desired setpoints be posted at the instrument so that the alarm setpoint can be readjusted to the desired position.

In addition, the data sheet has been revised to require that the person performing the calibration and instrument response check indicate that the alarm setpoint has been returned to the proper value. When reproduction of the procedures is complete, and procedures issued, appropriate individuals will be informed of the requirements of the revised procedures. This training will be completed by November 1, 1981.

This incident was also the subject of Licensee Event Report No. 81-03.
