OPERATING LI'ENSE DPR-54 DOCKET NO 50-312 IE INSPECTION REPORT N0 50-312/81-24 In reply to your inspection conducted by Mr. G. Hernandez on June 9 through June 12,1981, we offer the following information.

Appendix A of your letter notes the following deviation:

Appendix IB of the Quality Assurance Program as documented in Volume IV of the Rancho Seco FSAR, Paragraph 18.1.1 states in part that:

"All Class I structures, systems, and components must meet the applicable provisions of the Atomic Energy Conmission Quality Assurance Criteria for Nuclear Power Plants as set forth in the Code of Federal Regulations, Title 10, Part 50, Appendix B.

10 CFR50, Appendix B, Criterion V, states:

" Activities affecting quality shall be prescribed by documented instructions procedures, o-drawings of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.

Instructions, procedures, or drawings, shall include apprapriate cuantitative or qualitative acceptance criteria for determining that important activities have been satisfauorily accomplished."

Quality Assurance Procedure Number 20, Revision 1, of the SMUD Quality Assurance Manual states under Item 2 of the General Requirements that "The Quality Control Instruction shall be reviewed and approved by the Manager of Generation, Quality Assurance Director, and the Manager of Nuclear Operations.

R H ENGELKEN DIRECTOR September 1,1981 Contract Specification C22.2, Section 6, Paragraph SC-12, " Quality Assurance" states in part that:

" Work on this project which is identified as having a Quality Assurance Class 1... is to be performed under a written Quality Assurance Program pr9 pared by the involved contractor... and approved by the District's Quality Assurance Director prior to commencement of work."

Contrary to the above, on June 12, 1981, the inspector identified the following:

(1) Bechtel Quality Control was found to be utilizing a set of procedures entitled " Engineering Inspection Instructions" (EII) to perform their quality control function. The EII's were found to be neither described in the SMUD Quality Assurance Manual nor reviewed and approved by SMUD's District Quality Assurance Director.

U. S. Testing Company, the onsite testing laboratory, has been onsite since April 1981 performing quality related activities (soil, concrete sampling, testing, and inspec-tion) without a Quality Assurance Manual and quality control procedures approved by SMUD's District Quality Assurance Director.

This is a Feverity Level V Violation (Supplement II).

SMUD REPLY This District is in full compliance with both the above items as follows:

i (1) The District acknowledges that Bechtel Quality Control has had Engineering Inspection Instructions (EII) in prepara-tion for use as an aid in performing their quality control inspection function. The District's Quality Assurance Organization reviewed and approved the use of these docu-ments on August 11,1981 to be used in conjunction with the use of and as a supplement to Construction Iaspection Data t

Reports (CIDR's). This is in accordance with the SMUD Quality Assurance Manual, QAP No. 6.

Reference to the use of EII is now made in QAP No. 6, Inspection Planning, and in QAP No. 29, Contruction Inspection. The revised Quality Assurance Manual is to be issued in early. September,1981.

(2) The Quality Assurance Manual and Quality Control Procedures for Western States Testing, Division of United States Testing were reviewed in progressive stages prior to and after the Company's arrival onsite for compliance with 10CFR50 Appendix B requirements. The District acknowledges that approval did not occur.pcior to V. S. Testing's arrival on site to perform inspection / examination activities; however, the manual and procedures had been in the process of revision. Minor
