SUPPLY SYSTEM NUCLEAR PROJECT NO. 2 10CFR50.55(e) POTENTIALLY REPORTABLE CONDITION #120 CONTRACT 235 PROCUREMENT OF SAFETY RELATED EQUIPMENT LESS THAN IEEE STANDARDS In accordance with the provisions for reporting 10CFR50.55(e) conditions, your office was informed by telephone on October 14, 1980 of a potentially reportable condition regarding the procurement of safety related equipment to less than IEEE 323 requirements.
This condition has been evaluated and determined to be reportable. Attached is our final report on this matter.

r-ATTACHMENT 1 FINAL REPORT WASHINGTON PUBLIC POWER SUPPLY SYSTEM DOCKET NO. 50-397 LICENSES N0. CPPR-93 REPORTABLE DEFICIENCY AND CORRECTIVE ACTION FOR PROCUREMENT OF SAFETY RELATED EQUIPMENT TO LESS THAN IEEE 323 REQUIREMENTS SUPPLY SYSTEM NUCLEAR PROJECT NO. 2 Nature of Deficiency Approximately fifty (50) safety related relays and control switches were procured and installed without reference to environmental and seismic specifications required by IEEE 323.
Safety Implication These relays and switches are the control instrumentation for safety related switchgear. Their failure from extreme environment or seismic events could result in loss of ability to safely shut down the plant following a design basis accident.
CORRECTIVE ACTION Nonconformance Report (NCR) No. 235-05988 Revision 1 dated March 11, 1980 was issued against the General Electric Company, Installation and Service Engineering (GEI&SE) who purchased 'N relays and switches.
The NCR has been dispositioned to.myulre GEI&SE to:

Provide test data to backup Certificate of Compliance for the l
owner's review.
If the supporting data does not provide information that shows the devices to be qualified for Class lE service then the Contractor will be required to replace the existing devices with fully qualified devices.

Cancel orders for new devices and reorder devices from GE/NED with a requirement for environmental qualification in l
accordance with IEEE 323 1974.

Replace nonqualified Agastat relays with Agastat nuclear qualified E7000 series relays.
A Project Engineering Directive (PED) number PED-235-E-0108 has been issued to GEI&SE that provides all specifications for procurement of qualified devices.
