c Babcock & Wilcox Docket No. 99900262/81-01 NOTICE OF NONCONFORf1ANCE Based on the results of an NRC inspection conducted on June 22-24, 1981, it appears that certain of your activities were not conducted in accordance with NRC requirements as indicated below:
Criterion V of Appendix B to 10 CFR Part 50 states:
" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of r type appropriate to the circumstances and shall be accom-plished in accordance with these instructions, procedures, or drawings.
Instructions, procedures, or drawings shall include appropriate quantita-tive or qualitative acceptance criteria for determining that important activities have taen satisfactorily accomplished."
Nonconformances with these requirements are as follows:
A.
Paragraph 4.0 of Section 15 of the ASME accepted Quality Assurance Manual QAM-100 states in part, "An NCMR shall be prepared for all i tems not conforming to Company acceptance standards...."
Contrary to the above, an NCf1R was not prepared for Part No. 922501043, Body, Shop Order 21408-7-00, which contained a damaged weld prepara-tion that did not confonn to Company acceptance standards.
Prior to the exit interview the inspector was presented with NCMR No.
46444 that identified the damaged weld preparation of the body and the materials clerk had been instructed that QA/QC was to be notified when damage occurred in order for them to generate a NCMR.
Nonconformance closed, no further written response required.

Paragraph 3.0 of Section 12 of the ASME accepted Quality Assurance Manual QAM-100 states in part, "The system shall provide for periodic calibration of all measuring and testing equipment..
Paragraph " Calibration Intervals" of Quality Standard 1713-01 states in par'., "A recall system shall be maintained to assure the recall of all measuring system equipment used for inspection."
Contrary to the above, the system did not assure the recall for periodic calit' ration of all measuring equipment used for inspection, as evidenced by the observation on June 23, 1981, of a Scherr Tumico Optical Com-parator (No. 1179), which had an indicated calibration due date of April 23,1981, on the calibration sticker and had not been recalled or recalibrated.
