Contrary to Top' si Report CENPD-210-A and ANSI N45.2.11-1974, Quality Assurance of Dc Procedure 5.7, dated October 1, 1980 (which implements Sect n 9 of ANSI N45.2.11-1974), does not require that the design process and verification procedure be reviewed and modified as necessary when a significant design change is necessary because of incorrect design.

Cor.trary to Section 17.18 of Combustion E1gineering Topical Report CEN?D-210-A and Quality Assurance Procedure 18.1, a CE internal audit disclosed a nonconformance, but the cause and effect were not identified and the extent of the corrective action required was not determined.

OTHER FINDINGS OR COMMENTS 1.
Design Change Control - Inspection consisted of an examination of the review and approval records of the initial issue through the latest revision of:
(1) the Chemical and Volume Control System (CVCS) system description, (2) two CVCS design requirement documents, (3) four CVCS drawings, and (4) four CVCS specifications.
Applicable proceduras from the Quality Assurance of Design Procedure Manual were examined to determine the review and approval requirements for design changes.
It was determined that all changes had been pro-cessed in accordance with procedural requirements.

Design I..terfaces - Applicable procedures from tt3 Quality Assurance of Design Procedures Manual and the Plant Engine' ring Procedures Manual were examined to determine the design interface requirements and inter-facing organizations for each document.
The Locument Distribution /
Review and Document Comment Resolution forms for each revision of the design documents identified in D.1 above were examined for implementa-l tion of interface review requirements.
In addition, the records of l
external interface reviews by the A&E were examined for two CVCS specifications.
It was determined that interface review requirements had been implemented.

Design Corrective Action - Applicable procedures from the Quality Assurance of Design Procedures Manual were examined to determine the requirements for design corrective action.
It was determined that the procedures do not address the review of the design process and verificatica procedures when a significant design change is necessary because of incorrect design.
(See B.1 above).

Procurement Source Selection - Concerning nonconformance B.2. above, the recomended corrective action in response to the CE internal quality audit GQS-81-002 was to perfonn a quality evaluation of Fluidic Techniques (FT) relative to the Combustion Engineering Vendor Quality Control Program Specification WQC 11.1, Revision D, referenced in the procurement documents.
This evaluation survey of FT was conducted on March 3, 1981, and FT was found to have numerous quality assurance program deficiencies.
A hold was placed on all contracts at the vendor's facility until these deficiencies were addressed and corrected. A further review of past procurement history between CE and FT was made by the NRC inspector.
This review disclosed that two contracts for orifices had been placed, and delivery made to the Washington Public Power Service site prior to the CE evaluation survey at FT. No correspondence was reviewed by the NRC inspector which addressed the two contracts already shipped to the site.
No generic considerations by CE was evident concerning this internal finding.
Eight vendor files, eight procurement files, and five auditor qualifi-cation files were also reviewed and no other findings were identified in the programmatic implementation of procurement source selection.

Audits - To assure that procedural elements are being properly imple-mented, ten internal audits, six exter al audits, and auditor qualifi-cation files were reviewed and no adverse findings were identified.
