Appendix A NOTICE OF VIOLATION Immuno Assay Corporation License Nc. 21-17915-01 As a result of the inspection conducted on August 25, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), thc following violations were identified:

1.

License Condition No. 16 requires that licensed material be possessed and used in accordance with statements, representations and procedures contained in various dated applications and letters.

Item 5 of applications dated December 19, 1977 and Febr.ary 1,1978, name Thomas J. Kregoski as the radiation protection officer.

Contrary to the above, the individual designated as your radiation protection officer was changed and you failed to amend your license to reflect this change.

This is a Severity Level V violation (Supplement VII).

License Condition No. 16 requires that licensed material be possessed and used in accordance with statements, representations and procedures contained in various dated applications and letters. Letter dated January 9,1978 states that your Victoreen Model 493 survey meter will be calibrated at least every six months by the manufacturer.

Contrary to the above requirement, as of the date of the inspection, August 25, 1981, the last calibration of your Victoreen survey meter was on March 25, 1980, a period in excess of six months.

This is a Severity Level V violation (Supplement VII).

License Condition No. 16 requires that licensed material be possessed and used in accordance with statements, representations and procedures contained in various dated applications and letters. Application dated December 19, 1977, supplement to Item No. 14, states that a GM survey must be performed in lab areas once each week.

Appendix A Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each item of noncompliance:

(1) cor-rective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation. Consideration may be given to extending your response time for good cause shown.
