APPENDIX A NOTICE OF VIOLATION PF.iladelphia Electric Company Dock't No.
50-353 Limerick Generating Station, Unit 2 License No. CPPR-107 As a result of the inspection conducted on July 1-24, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violation was identified:
10 CFR 50, Appendix B, Criterion IX, states, in part, that:
" Measures shall be established to assure that... welding... (is) controlled and accomplished... in accordance with applicable codes, standards, specifica-tions, criteria, and other special requirements."
The Limerick PSAR, Appendix D, paragraph D.4.3, " Control of Special Processes, Tests and Inspections," states, in part, that:
"The vendor shall operate under a controlled manufacturing system and shall have written procedures or instructions for control of... welding..."
The pipe drawing DBB-203-1, Revision 3, Note 4.B, states, " Grind ID of buttweld flush (where accessible) and OD per detail 'D'."
Contrary to the above, on July 14, 1981, vendor shop weld No. 3, on pipe spool DBB-203-1, was not ground flush on the inside diameter.
This is a Severity Level V Violation (Supplement I ), applicable to CPPR-107.
Pursuant to the provisions of 10 CFR 2.201, Philadelphis; Electric Company is hereby required to submit to this office within thirty d4ys of the date of this Notice, a written statement or explanation in reply,\\ including:
(1) the corrective steps which have been taken and the results achieved; (2) corrective stepswhichwillbetakentoavoidfurtherviolations;and(3)thedatewhen fell compliance will be achieved.
Under the authority of Section 182 of the AtomicEnergyActof1954,asamended,thisresponseshallgesubmittedunder oath or affirmation. Where good cause is shown, considerataon will be given to extending your response time.
