  • iOTICE OF VIOLfTION Pathology Associates Medical l aboratories License 'Jo. 51-07993-03 Chemistry Departoont 4400 Kalanianole Hiqhway Honolulu, Hawaii 96816 As a result of the inspection conducted on July 30 and 31,1981, and in accordance with the Interim Enforcenent Policy, 45 FR 66754 (October 7, 1930), the following violation was identi fied:

License Condition 12. states that licensed naterial shall be used by, or under the supervision of, Paul Y. Tamura, ". 3.

Contrary to this rergirement, at the tine of the inspection, Dr. Tamura had left Pathology Associates effective May 31, 1981 and licensed materials were beinq used by unauthorized individuals.

This is a Soverity level IV Violation (Supplement VII).

Pursuant to the provisions of 10 rFP 2.201, Pathology Associates Madical Laboratories, Honolulu, Hawaii, is hereby required to submit to this office w' thin thirty days of the date of this Notice, a written statement or explanation in reply, including:

(1) the corrective steps which have been taken and the rosults achieved; (2) corrective steps whico will be taken to avoid further items of noncompliance; and (3) the date when full connliance will be achievod.

Consideration may he given to extending your response time for good cause shown.
