NOTICE OF VIOLATION The.Childrens Hospital License No. 34-03111-02 As a result of the inspection conducted on August 20, 1981, and in accord-ance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980),

the following violation was identified:

License Condition No. 20 states that the licensee shall possess and use

.l Pensed material in accordance with statements, representations and pro-cedu.es listed in the application dated December 27, 1977.

Item No.10 of the application requires that the linearity test of your dose calibrator be carried out over 48 hours5.555556e-4 days <br />0.0133 hours <br />7.936508e-5 weeks <br />1.8264e-5 months <br />.

Contrary to the above, linearity tests performed on your dose calibrator have only been carried out over 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />.

This is a Severity Level V violation (Supplement VII).

Pursuant to the provisions of 10 CFR 2.201, you are required to subsi+ to this office withia thirty days of the date of this Notice a written statement or explanation in reply, including for each item of noncompliance:

(1) cor-rective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall ba submitted under oath or affirmation. Consideration may be given to extending your response time for good cause shown.
