Dear Mr. O'Reilly:
On August 24, 1981,'a significant deficiency as defined by 10CFR50.55(e) was reported to Mr. Virgil Brownlee at the NRC Region II office. The item involved an improper termination to a solenoid valve that controlled an air operated containment isolation valve. The termination was not made per approved procedure and could have failed. Details are given on the attachment.
This item is being reported under our 10CFR50.55(e) procedure. This is considered the final report on the matter.

10CFR50.55(e) - SIGNIFICANT DEFICIENCY 1.
Identification of Nonconformance An improper termination was made to a solenoid valve that controls an air operated valve that serves as containment isolation for the Reactor building air compressor suction. The termination was not made per procedure and could have failed.

Number and Location of Nonconformance The nonconformance was an isolated incident limited to the one solenoid.

Significant Laficiency Created and Evaluation Failure o' the termination could cause a short or an open circuit that could prevent the solenoid from energizing. This could result in a violation of containment isolation.
4.
Corrective Action The solenoid was reterminated per approved procedure. The maintenance personner responsible for the incident is no longer employed.
