This refers to the routine safety inspectbn.,nducted by Mr. R. E. Burgin, Dr. W. J. Adam and Ms. E. R. Matson of this office on August 5-7, 1981, of activities at Ohio State University authorized by NRC Byproduct Material Licenses No. 34-00293-02, No. 34-00293-08 and No. 34-00293-12 and to the discussion of our findings with Dr. Ronald L. St. Pierre, Mr. George L. Hahn, Dr. John Olsen, Dr. William Ploughe, Mr. Jerry Dare and Mr. Gene Roe at the conclusion of the inspection.
The inspection was an examination of activities conducted unoer your license as they relate to radiation safety and to compliance with the Commission's rules and regulations and with the conditions of your license. The ir. spec-tiori consisted of a selective examination of procedures and representative records, observations, independent measurements, and interviews with personnel.
In addition to the above areas, the inspectors examined actions described in your letter dated August 21, 1978, regarding apparent items of noncom-pliance found during our June 19-23, 1978 inspection.
Both items identified during the previous inspection were found as repeat items cf noncompliar.ce during this inspection.
During this in:,pection, certain of your activities appeared to be in non-compliance with NRC requirements, as specified in enclosed Appendix A.
A written response, submitted under oath or affirmation, is required.
