This refers to the routine safety inspection conducted by Messrs.
W. P. Reichhold and J. R. Mullauer of this office on August 5,1981, of activities authorized by NRC Byproduct Material License No. 1.2-01678-01 and No. 12-01678-02 and to the discussion of our preliminary findings with you on August 5, 1981, and the telephone conversation with you on August 12, 1981.
The inspection was an examination of activities conducted under your license as they relate to radiation safety and to compliance with the Commission's rules and regulations and with the conditions of your license.
The inspection consisted of a selective examination of pro-cedures and representative records, observations, independent measure-ments, and interviews with personnel.
In addition to the above areas, the inspectors examined actions described in your letter dated September 14, 1977, regarding two apparent items of l
noncompliance found during our August 25, 1977 inspection. We have no further questions regarding these matters.
During this in;pection, certain of your activities appeared to be in non-compliance with NRC requirements, as specified in enclosed Appendix A.
l A written response, submitted under oath or affirmation, is required.
i In addition to our concern for corrective action with respect to the specific items of noncompliance noted, we are concerned about your management control system that has permitted these items to occur.

i Fayette County Hospital AUG 2 51981 Consequently, in your reply to this letter, please describe those actions taken or planned to improve the ef fectiveness of your manage-ment centrol over your licensed activities so as to preclude the recurrence of these or similar items of noncompliance.
Enclosed is Appendix B, " Management Control" to provide you with some guidance for your response.
We will gladly discuss any questions you have concerning this inspection.
