Please find enclosed the documents listed on Appendix A.

The charge for reproducing records is five cents ($0.05) per page, as specified in 10 CFR 9.14(b).
Accordingly, the cost of reproducing the enclosed 320 pages is $16.00.
Please send a check made payable to the U. S. Nuclear Regulatory Commission in the amount of $16.00, along with a copy of this letter, to the Director, Division of Rules and Records, Room MNBB-4210, U. S. Nuclear Regulatory Commission, Washington, DC 20555.
Portions of document six of the appendix have been deleted in order to withhold the names of persons who have been exposed to radiation.
Because disclosure of this information would constitute a clearly unwarranted invasion of personal privacy, it is being withheld from public diclosure pursuant to Exemption (6) of the Freedom of Information Act (5 U.S.C. 552(b)(6)) and 10 CFR 9.5(a)(6) of the Commission's regulations.
Pursuant to 10 CFR 9.9 of the Commission's regulations, it has been determined that the information withheld is exempt from production or disclosure, and that its production or disclosure is contrary to the public interest. The persons responsible for this denial are the undersigned and Mr. Victor Stello, Director, Office of Inspection and Enforcement.
This denial may be appealed to the Commission's Executive Director for Operations within 30 days from the receipt of this letter. As provided in 10 CFR 9.11, any such appeal must be in writing, addressed to the Executive Director for Operations, U. S. Nuclear Regulatory Commission, i
Washington, DC 20555, and should clearly state on the envelope and in j
the letter that it is an " Appeal from an Initial F0IA Decision."
