SUMMARY
OF OPERATING EXPERIENCE t
i l-OPERATION:
l The month began with the plant in cold shutdown. Heat up commenced July 2, 1981.
-The plant entered Mode 3 on July 3, 1981. As temperature and pressure approached NOT & NOP evidence of a failed inner Reactor Vessel seal caused. personnel to shift to the outer 0-Ring seal. An unusual event (RERP) was declared July 3, 1981, when l
RCS leakage exceeled STS limits due to A, B, and C RCS loop drains which had to be
-torqued shut. Reload Physics Testing started July 4, 1981, with the plant enter-i ing Mode 2 the same day. On July 10, 1981, the Reactor was manually tripped when 1
'B' RCP lost oil to its lower bearing. The plant was' cooled down to 360*F while repairs were made to the pump. The Reactor was taken critical again on July 17, l
1s81, but was shutdown to Mode 2 on July 18, 1981, due to high cation conductivity in the steam generator.
The plant returned to power July 19, 1981, and went on to complete Reload Physics Testing July 22, 1981. On July 30, 1981, the Reactor' tripped on 'B' steam generator lo-lo level when a utility worker inadvertently caused 'B' MSIV to shut while c1 caning on the main steam support structure. The plant was recovered and closed out the month on-line producing power.
MAJOR SAFETY RELATED MAINTENANCE:

l Work continued on improvement modifications to the Plant Security and Fire Pro-l tection Systems.
Vork continued on modifications of walls to improve seismic integrity.
I Work continued on several TMI-related design modifications.'B' RCP lower radial i
bearing replaced after loss of oil in lower reservoir.
