Evaluation of the significance of Potentially Reportable Incident No.

46 has been completed in accordance with 10CFR50.55(e) and Company Porcedure N-23 and is considered reportable.

Hereafter, it will be referred to as Significant Construction Deficiency No. 34.

We are attaching herewith a suggested draft letter and Interim Report from LP&L to the US NRC.

Ebasco recommends that this Interim Report be filed with the US NRC by August 7, 1981.

INTERIM REPORT OF 3

SIGNIFICANT CONSTRUCTION DEFICIENCY NO. 34

" BUFFALO FORGE HVAC FAN-SHROUDS" INTRODUCTION-This report is submitted persuant to 10CFR50.55(e). It describes a deficiency in the shrouds'to serve as fan blade missile barriers.

DESCRIPTION The attached references state that the shroud thicknesses on some fans pur-chased from the Buffalo Forge Company would not contain a thrown fan blade.

This condition, if lef t uncorrected, could jeopardize the functional oper-ability of other. nuclear safety related equipment in the vicinity. The fan housing thickness must be corrected to the acceptable level in order that if a fan blade were to throw, it would not penetrate the housing and become a missile.

SAFETY IMPLICATIONS If a fan blade penetrates its shroud and becomes a missile, other safety related equipment in the area could be damaged by this missile.

This missile damage could lead to loss or degradation of the affected safety related equipment. Thus, the equipment could be unable to carry out its re-quired safety function.

1 Therefore, the inadequate missile barrier design of the shrouds would ad-versely affect the safety of the plant if left uncorrected.

CORRECTIVE ACTION The Buffalo Forge Company is proceeding with drawings of suggested means of field modifications for the fans in question. Drawings are expected to be mailed to Ebasco no later than August 7, 1981.

After drawings are reviewed, Ebasco will advise as to the anticipated date l

of corrective action.

On June 5, 1981, we made a telephone rep' ort to Mr. Robert Bear and Mr. William Mills which they considered to be our pre.

liminary report under 10CFR Part 21.

Our final report is as follows.

We have a number of contracts which specify that missile penetration analyses be performed.

Those contracts for which we have either shipped all or part of the equipment are listed below.

During the course of. executing these contracts we per-formed the necessary missile penetration analyses and designed the equipment accordingly.

Some of the contracts were for centrifugal fans while others were for axial-flow fans.

New methods of calculation indicate thicker housings.

4 We are not in a position to judge whether any missiles which might penetrate the housings would produce a condition which would be classified as a substantial safety hazard.
