[7b AT'IN:

Mr. William York President 5250 Mayfair Road

.Nurth Canton, OH 44720 Gentlemen:

Tnis refers to the routine ' safety inspection conducted by Mr. W. J. Slawinski of this office on June 3,1981, of activities at your North Canton, Ohio facility authorized by NRC Byproduct Material License No. 34-14071-01 and to the discussion of our findings with Mr. A. C. Magno at the conclusion of the inspection.

The inspection was an examination of activities conducted under your license as they relate to radiation safety and to compliance with the Commission's rules and regulations and with the conditions of your license. The inspet-tion consisted of a selective examination of procedures and representative records, observations, independent measurements, and interviews with personnel.

l l

In addition to the above areas, this inspection included a review of the incident involving the theft of one af your vehicles containing an exposure device, as documented in your letter dated February 11, 1981. We have no further questions regarding this matter.

Also reviewed were the actions described in your letter dated September 9, 1980, regarding apparent items of noncompliance found during our February 11 and July 29-30, 1980 inspections. On the basis of information included in your September 9,1980 letter, it appears tha; the requirements of 20.101(b) were met before the 1 32 rems exposure to one of your radiographers. This information was not made available to our inspector during the February 11 i

and_ July 29-30, 1980 inspections.

You should ensure that your records system allows retrieval of NRC required documentation during future inspections.

l Our records will be modified to delete the overexposure.

i-During this inspection, certain of your activities appeared to be in noncompliance with NRC requirements, as specified in enclosed Appendix A.

A

'witten response, submitted under oath or affirmation, is required.
