This refers to the routine safety inspection conducted by Ms. E. Matson, Mr. J. Lynch and Mrs. R. Douglas of this office on July 22 and 27,1981, of activities authorized by NRC Byproduct Material Licenses No. 12-01257-04 and No. 12-01257-05 and to the discussion of our findings with you and members of your staff at the conclusion of the inspection.
The inspection was an examination of activities conducted under your license as they relate to radiation safety and to compliance with the Commission's rules and r.gulations and with the conditions of your license.
The inspection ccasisted of a selective examination of procedures and representative records, observations, independent measurements, and interviews with personnel.
During this inspection, certain of your activities appeared to be in non-compliance with NRC requirements, as specified in enclosed Appendix A.
A written response, submitted under oath or affirmation, is required.
This inspection also included a measurement of the output of your teletherapy equipment. Our measurement was made using a Victoreen Mod :1570 Condenser R-Meter with Model 621 chaeber, calibrated by the National Measurements Laboratory. The source-to-chamber distance was 80 centimeters and the field size used was 10 by 10 centimeters. After applying standard correction factors for temperature, pressure, timer error, roentgen to rad conversion, and attenuation, our measurement of the output of your teletherapy equipment was 112.62 rads per minute. The most recent full calibration of your tele-therapy equipment, conducted on December 23, 1980, reported a measured output for the same exposure parameters of 118.38 rads per minute.
Applying a correction factor to account for seven months and for days decay, the value calculated from the December 23, 1980, calibration becomes 109.47 rads per minute.
