Appendix A NOTICE OF VIOLATION Analytical Bio Chemistry License No. 24-13365-01 Laboratories, Inc.
As a result of the inspection conducted on July 17, 1981, and in accordance v2th the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violations were identified:
1.
License Condition No. 16 requires that licensed material be possessed and used in accordance with statements, representations, and procedures contained in application dated July 20, 1979.
The license application referenced at ve states that the survey instruments will be calibrated ar;.iually.
Contrary to this requirement, your Ludlum G-M survey meter Serial 9518 Model 3 was last. calibrated on January 23, 1980.
This is a Severity Level V violation (Supplement VII).

10 CFR.71.5(a) requires that no licensee shall transport any licensed material outside of the confines of his plant. or other place of use unless the-licensee complies with the applicable regulations of the Department of Transportation in 49 CFR Parts 170-189.
49 CFR 173.391(a)(4) states that no significant removable radioactive contamination shall be on the external surface of packages.
I Contrary to this requirement, wipe tests were not performed to ensure that no removable radioactive contamination was present on packages shipped, since the date of license issuance.
This is a Severity Level V violation (Supplement V).
3.
10 CFR~20.207(a) requires that you secure licensed materials stored in an unrestricted area against unauthorized removal.
Contrary to this requirement, on July 17, 1981, five barrels containing approximately 9.3 millicuries of carbon-14 were stored in an unlocked shed and accessible to unauthorized individuals.
This is a Severity Level V violation (Supplement IV).

Pu,suant to the provisions of 10 CFR 2.20', you are required to submit to this office within thirty days of the date of this Notice a written state-meat or explanation in reply, including for each item of noncompliance:
('.) corrective action taken and the results achieved; (2) corrective t.ction to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation. Consideration may be given to extending your response time for good cause shown.
