. Appendix A NOTICE OF VIOLATION

Wooddell Logging, Inc.

ILicense No. 12-19038-01 As a result of the inspection conducted on July 21 and 22, 1981, and'in accordance with the Interim Enforcement Policy, 45 FR'66754 (October 7, 1980), the following violation was identified:

License Condition No. 18. requires that licensed material be possessed and used in accordance with statements, representations and procedures contained in. application dated May:10, 1979 and letter dated November-16, 1979.

Included in the application (Item No. 11 a) is a requirement to calibrate survey meters every six months.

Contrary to the above requirement,on the day of the inspection-records showed that two survey meters had.not been calibrated within'the six month interval.

Instrument No. 3895 was calibrated November 28, 1980; instrument No. 6530 was calibrated July 22, 1980. Both instruments were used until July 3, 1981, a period greater than six months.

This is a Severity Level V violation (Supplement VII).

Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written' state-ment or explanation in reply, including for ea~ch'icem of noncompliance:

(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved. Under the ' authority of Section 182 of the Atomic Energy Act of 1954, as amended,-this response shall be submitted under oath or affirmation. Consideration may be given to extending your response time for good cause shown.
