Appendix A NOTICE OF VIOLATION I

As a result of the routine inspection conductedlon July 15, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980),.the following violation was identified:
License Condition ~ 14 requires that all : licensed material be possessed and..
used in accordance with statements, representations.and procedures contained' in the applications' dated April'23i 1980 an'd January 7, 1981,-and the letter dated May 22, 1980.
Item 17'of the April 23, 1980' application states that all elution, pre-paration and injection areas will be. surveyed ~ daily with a G-M survey meter and decontaminated if necessary.
Item 10 of this same application states that when a survey meter is sent for repair or calibration, a
" loaner" survey meter will be supplied by Health Physics-Associates Ltd.
Contrary to the above,.from June 30, 1981 to the date of this inspection,-
no Gh' survey meters were available at your facility, and therefore, no -
daily radiation area surveys'were performed during:this same period.
This is a Severity Level'V violation (Supplement'VII).
Pursuant-to.the provisions of 10 CFR 2.201,.you are required to submit.to this office within thirty days of the 'date of this Notice a written state-ment or explanation in reply, including for each item of noncompliance:
(1) corrective action taken and the results achieved; (2) corrective.
action to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954,'as amended, this_ response shall be submitte'd i
