50-358, CONSTRUCTION PERMIT NO. CPPR-88, W. O. #57300-957, JOB E-5590 Gentlemen:

Our statement of explanation for the item of noncompliance cited in Appendix "A" of report 81-15 is as follows:

(1) .Cy rective Action Taken and Results Achieved Immediate corrective action was initiated on the two identified violations of procedural requirements:

(a) The I&C group immediately ceased hanging green jurisdictional tags on instruments. These will be hung by operations or rad / chem group personnel in accordance with the SU.PRP.12 recuirements, and (b) Upon learning the scope and magnitude of the maintenance work being performed, the Station Quality Engineer revised the QC requirements on the subject work request (WR 1331). The job then continued with proper QC surveillance.

(2) Corrective Action to Be Taken to Avoid Further Noncompliance Corrective action to be taken on the two identified violations of procedural requirements is:

(a) The 1&C Engineer will instruct the I&C Foreman in the requirements of SU.PRP.12 which has been revised to allow the I&C group to place jurisdictional tags relating to instrumentation.
