As a result.of the inspection conducted on July 23, 1981, and in accordance 1

with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violations were identified:

License Condition No. 17 requires that licensed material be possessed and used in accordance with statements, representations and procedures con-I' tained in the application dated November 25, 1980. The section of the application procedures entitled "G. Training Procedures - I. Assistant' Radiographer, Page 12" outlines the training required before an individual can function' as an ' assistant radiographer.

Contrary to this requirement, an individual functioned as an assistant radiographer without completing your training program. Specifically, one individual had not been given the assistant radiographers test nor been instructed in pertinent NRC Title 10 regulations.

10 CFR.20.401(a) requires that you' maintain records showing radiation exposures on Form NRC-5, in accordance with the instructions contained-in that form or on clear and legible records _containing all the infor-mation required by Form NRC-5.

Contrary to this requirement, on-the day of the inspection, July 23, 1981, your exposure records did not contain the required information. Specifi.

cally, they did not'show social security numbers and birthdates of indi-viduals for which film badge monitoring is required.

This is a Severity Level VI violation (Supplement IV).

-Pursuant to the-provisions of 10 CFR 2.201, you are required to submit to this.

office within thirty de.ys of the date of this Notice a written statement or-explanation in reply, including for each item of noncompliance:

(1) corrective action taken and the results achieved; (2) corrective action-to be taken to avoid further noncompliance; and (3) the date when full compliance will'be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation. Con -

sideration may be given to extending your response time for good cause.shown.
