Robert it. Bernem, Director Division of Systems & Reliability Research Office of fiuclear Regulatory Research

SUBJECT:

LER TREtiD; HATCH 1. HATCH 2 This meno is ir. tended to call your attention to the Hatch 1 and 2 units.

He have noted an LEP, trend that gives us some concern that coincidental multiple system failures of importance are more likely to be seen at these units than at others.

If you are not already doing so, we urge you to follow closely the operational experience with these units and those actions being taken to impmve the situation.

As you know, our systems people mutinely peruse the Pfis and the LER cards from NSIC.

He do this to gain insight on events, failure data, precursors, and trends of particular interest to our development of PRA techniques. Me also follow these as part of the job of reviewing or identifying candidate events for the abnomal occurrence report to Congress.

In our recent perusal of LERs covering mughly the period of spring through fall of 1980, we observed that relative to other BWRs, an unusually high frequency of multiple fault or potentially interactive events was i

being experienced by the Hatch units.

These events have involved vital standby systems needed for high and low pressure coolant makeup and for shutdown heat removal (e.g., RHR, P,CIC, HPCI, RHR-service water and room 8108130347 810608 i PDR FOIA t

MADDEN 81-159 PDRj I

cooling support systems, etc.). As you can also observe from the enclosed sum.ary chart, a nuaber of the Match etents seem to be_repeti-tive in nature. Viewed individually, each LER may not seem of great significance; taken together, however, these da suggest some systematic difficulties needing irrprovement.

g Please advise if we can be of any assistance in this matter.
